Comprehensive Physical and Cognitive Assessment: This is the fundamental piece that helps us understand the “big picture” of a client and their individual needs
Home Safety Evaluations: Evaluate the physical layout for potential fall risks and options for modifications
Medication Management: Set up and monitor medication compliance. Monitor effectiveness and observe for side effects of any new medications or changes in medication dosages
Relocation Assistance: Helping determine the level of care needed and the best type of placement. Also available to tour facilities and assist with moving related issues
Physician/Therapy appointments: Assist with scheduling, keeping all involved practitioners informed on treatment plans, and even attending appointments with our clients
Emotional Support: Listen actively to concerns, feelings, frustrations that evolve during these situations, for our clients and their support persons. Promote independence to the greatest degree possible
Caregiver Education: Educate support persons, caregivers, and clients regarding illness and disease prevention and processes, medications, treatment plans, etc.
